¿Qué es la diabetes gestacional?

Gestational diabetes is a type of Diabetes that first occurs during pregnancy, usually toward the end of the second trimester. This condition causes your body to be resistant to insulin, leading to high blood sugar levels.

Gestational diabetes usually goes away after giving birth but can increase your chance of developing Type 2 diabetes later in life. Seeking treatment as soon as possible helps keep you and your baby healthy.

Gestational diabetes symptoms 

Many women with gestational diabetes do not have symptoms. You might only find out you have it after a screening test your obstetrician-gynecologist (OBGYN) will perform between 24 and 28 weeks of pregnancy.

However, it can cause symptoms of high blood sugar, including:

  • Bladder infections
  • Visión borrosa
  • Feeling more thirsty than usual
  • Micción frecuente
  • Cansancio

Some of these are common pregnancy symptoms, so they do not necessarily mean you have diabetes. However, a test can make sure you get early treatment for gestational diabetes to help ensure a healthy pregnancy.

What causes gestational diabetes?

Diabetes occurs when your body does not make enough or becomes resistant to insulin, the hormone that regulates blood sugar.

The exact cause of gestational diabetes is not known. One theory is that hormones produced by the placenta can block the action of the mother’s insulin and lead to insulin resistance. Gestational diabetes can also develop when the body cannot make the insulin it needs. Without insulin, sugar is not converted to energy and can build up in the blood.

What are the complications associated with unmanaged gestational diabetes?

Uncontrolled gestational diabetes can result in high blood sugar levels, which may cause complications for both you and your baby. Including:

  • Increased birth weight (macrosomia): High blood sugar can cause your baby to store excess fat, leading to a large baby, which may cause delivery complications like shoulder injury.
  • Infant health complications: Your baby may have low blood sugar (hypoglycemia) after birth and a higher risk of diabetes later in life. Early birth is more likely, potentially causing breathing issues and developmental delays.
  • Cesarean section: Gestational diabetes increases the likelihood of needing a Cesarean section, particularly with a large baby. This surgery carries risks and requires a longer recovery time.
  • Preeclampsia: There's a higher risk of preeclampsia, a condition causing high blood pressure and potential preterm delivery. Your doctor will closely monitor you.
  • Type 2 diabetes risk: Gestational diabetes often resolves after childbirth, but about one in two women develop Type 2 diabetes later. Regular blood tests and a healthy lifestyle can help reduce this risk.

Gestational diabetes risk factors

Gestational diabetes impacts 2% to 10% of pregnancies in the United States. Risk factors for gestational diabetes include:

  • Age, race and ethnicity: Higher risk if over 35 years or if Black, Hispanic/Latino, American Indian, Alaska Native, Native Hawaiian or Pacific Islander.
  • Overweight: Increased risk if overweight/obese before pregnancy or if you gain excessive weight during pregnancy. Discuss healthy eating and weight management with your doctor.
  • Current health: Sedentary lifestyle and high triglyceride levels may increase risk. A healthy lifestyle can lower it.
  • Previous pregnancies: Higher risk if you had gestational diabetes or a large baby in previous pregnancies.
  • Family history: Genetic factors may contribute to gestational diabetes. Risk is higher if a close relative has diabetes.
  • Polycystic ovary syndrome (PCOS): Women with PCOS have a greater chance of developing gestational diabetes and other pregnancy complications.

How is gestational diabetes diagnosed?

Screening for gestational diabetes includes blood tests to monitor your blood sugar levels and usually occurs before the 28th week of pregnancy. If you are at high risk for gestational diabetes, your doctor might recommend earlier testing.

  • Initial glucose challenge test: For this type of gestational diabetes test, you will drink a liquid containing glucose (a type of sugar) and have your blood drawn one hour later. If your blood sugar level is elevated, you may need to take a follow up glucose tolerance test.
  • Follow-up glucose tolerance test: After fasting (not eating) for at least eight hours, you will have a blood test to check your blood sugar levels. Then, you will drink a glucose liquid and have additional blood tests every hour for two to three hours. Your doctor will explain what your results mean.

Gestational diabetes treatments

If you have gestational diabetes, your healthcare provider will schedule more frequent checkups to closely monitor both you and your baby. You may undergo tests such as a nonstress test and a biophysical profile to ensure you and your baby are healthy.

A diagnosis of gestational diabetes might seem worrying, but you can manage the condition with lifestyle changes, blood sugar monitoring and medication.

Your lifestyle—diet and physical activity—plays a key role in managing your blood sugar levels. While weight loss during pregnancy is generally not recommended, as your body supports your growing baby, your healthcare provider can help set appropriate weight gain goals based on your pre-pregnancy weight.

Lifestyle changes include:

  • Changes to your diet: Diet is an important way to control diabetes. You can work with a dietitian to develop a healthy meal plan and learn how to manage your blood sugar levels.
  • Exercise: Staying active can reduce your blood sugar levels and make your body more sensitive to insulin. If you are new to exercising, ask your healthcare provider for advice. Most people should aim for 30 minutes of moderate-intensity activity most days of the week.

How can I prevent gestational diabetes?

Preventing gestational diabetes isn't guaranteed, but adopting healthy habits before pregnancy can help. If you've had gestational diabetes, these choices may lower your risk in future pregnancies or for developing Type 2 diabetes:

  • Eat healthy foods: Choose high-fiber, low-fat and low-calorie options. Focus on fruits, vegetables and whole grains. Variety helps you meet your goals without sacrificing taste or nutrition. Be mindful of portion sizes.
  • Stay active: Regular exercise before and during pregnancy can reduce your risk. Work towards 30 minutes of moderate activity most days. Brisk walking, cycling and swimming are great options. Short bursts of activity, like parking farther away or taking walk breaks, also count.
  • Start pregnancy at a healthy weight: If you're planning to conceive, losing excess weight beforehand may lead to a healthier pregnancy. Develop lasting healthy eating habits, like increasing your vegetable and fruit servings.
  • Manage weight gain: Some weight gain during pregnancy is normal, but gaining too much too quickly can raise your risk of gestational diabetes. Talk to your healthcare provider for personalized weight gain recommendations.
